The Physician Must Set Up An Account

Cryogenic Laboratories, Inc. (CLI) does require that a licensed physician/practitioner, if he/she has not already, establish an account with us before sperm specimens are ordered and shipped. The Physician Account Set Up Form is available by contacting Client Services at 800-466-2796.

The Patient Must Complete and Return the Terms of Use Letter

Effective November 1, 2006 for new patients, the Terms of Use Letter must be signed by the patient and returned to us prior to shipment of any donor specimens. The form does NOT need the signature of the physician. The form may be returned by fax, mail or email. Contact Client Services for assistance if needed. The Terms of Use Letter explains what to expect from our semen donor program and what we expect from the patient when purchasing and utilizing our services. Please note that if an ID Option donor is chosen, the patient will be required to complete and return a signed ID Option Patient Agreement. This form is donor specific and will need to be mailed, faxed or scanned and emailed at least one business day before placing the order.

The Physician Or Patient Places the Order

Either the patient or healthcare provider may place an order. If an order is to be billed to the physician's office a staff member must place the order. Before placing an order, please check availability online or by calling Client Services. Also, please ask the patient to review all available donor information. You may place an order by contacting Client Services at 800-466-2796. Additionally, CLI provides online ordering for donor sperm and additional information through the Donor Search.

At the time of ordering please be prepared to provide the following information:

• Patient name, address, and phone number
• Medical Professionals name and shipping address
• Donor number
• Number of units and preparation type (Pre-washed IUI, Standard ICI, IVF)
• Date the units need to arrive at the provider's office
• Payment information

CLI accepts all major credit cards, money orders and cashier checks. Personal checks are not accepted. CLI must receive any money order or cashier check before the shipping date. CLI does not work with or arrange payment with insurance companies, although we can provide the patient with copies of paid invoices to submit to insurance upon request.

Client Services takes orders 7am to 6pm Monday - Friday, Eastern Time (ET).

Deliveries are made on business days only. Should there be an exceptional circumstance, CLI will do all that is possible to accommodate the patient and/or provider. CLI offers a courtesy hold for units up to 10 business days. All hold orders will be cancelled on the tenth business day if the order has not been completed.

CLI's standard shipping tanks are guaranteed to maintain frozen samples up to 7 days. Upon request and an additional fee, a 14 day tank can be shipped, which guarantees the samples stay frozen up to 14 days.

For Fed Ex Orders:

All non-local shipments (outside of the Minneapolis metro area) are transported using FedEx and are delivered directly to the health care provider's office or the patient's home address (if the Home Delivery Authorization Form has been completed by the physician). Orders must be complete before 2pm ET on the day shipment is required. A rush fee will be accrued for orders placed between 2pm ET and 3pm ET. Orders placed after 3pm ET cannot be shipped the same day.

For Local Courier Orders:

All local shipments (in the Minneapolis/St. Paul area) are transported using a local medical courier and are delivered directly to the health care provider's office or the patient's home address (if the Home Delivery Authorization Form has been completed by your physician). Orders must be complete before 11am ET on the day shipment is required.

For Internet Orders:

All Internet donor sperm orders must be placed  at least one business day PRIOR to the day of shipment. Physician accounts must also be set up at least one business day prior to ordering. Confirmations of web orders can be requested at the time of ordering.
